How Can I be a Care Provider?

Eddie Guardado Foundation (EGF) grants provide financial relief for families who have children with medical needs not covered or not fully covered by their commercial or government health benefit plan. The Foundation aims to fill the gap between what medical services/items a child needs and what their health benefit plan will pay for.
How Does the Grant Work?
If a grant is approved by the EGF Board of Directors for a child, the grant will help pay for approved medical services/items.
How Do I Work With a Family Applying For a Grant?
The family will need a signed letter from the health care provider (you) which defines their medical condition. The health care provider/practitioner must support the condition for which the family is requesting help. The family will need to know what the cost of the provided medical services/items are and what their out of pocket cost will be so they can indicate it on their grant application. The family must apply for the grant themselves.
How Do I Work With an Approved Family?
If a health benefit plan has submitted payment to you, please send any remaining amount due to the family in the form of an invoice, not a health insurance claim form. The Foundation is not a secondary insurance plan. If the family has no coverage for a requested service/item, please send an invoice to the family. The family will forward to the Foundation and we will pay you directly. The Foundation will not pay “provider responsibility” amounts as listed on the family’s explanation of benefits document.
In all cases, grant money disbersed to the healthcare provider will be applied as a credit to the family’s account with that provider. The healthcare provider will provide a monthly account statement to EGF indicating invoices for services and the amount of the credit applied from the grant.
